Grace Paradela, M.D.

Grace Paradela, M.D.

Board Certified in Internal Medicine

Dr. Grace Paradela completed her medical studies in the Philippines. She graduated magna cum laude in BS Medical Technology as her undergraduate degree. Dr. Paradela interned at the Philippine General Hospital, which happens to be the largest tertiary referral hospital in the country. Dr. Paradela has worked in multiple rural areas of the country, working with pediatrics, minor surgery, family practice, and Obstetrics and gynecology. After fulfilling her residency in Internal Medicine in New Jersey, she moved to St. George in 2002. She has been practicing acupuncture since 2006. She greatly values the integration of both western and eastern disciplines. Dr. Paradela is board-certified in Internal Medicine, board eligible in acupuncture and obesity medicine. She enjoys family time with her husband and her two sons and two furbabies. She enjoys poetry, landscape and flower photography, bagmaking, karaoke and crime TV.

Jonathan Baza, DO

Board Certified in Family Medicine

Dr. Jonathan Baza is originally from Bountiful, Utah. His undergraduate degree was in Athletic Therapy, at Weber State University in Ogden Utah. He graduated from medical school in 2016 from Touro University Nevada. He completed a residency at Southeastern Health in Lumberton, North Carolina. Dr. Baza received his medical degree in Osteopathic Medicine and had residency training in Family Medicine. He uses Osteopathic Manipulative Treatments (OMT) to treat a variety of sports and chronic injuries. His medical interests include: sports medicine, dermatology, and women’s health. Dr. Baza is married and has three children. He and his family love playing games together and traveling, pickle ball, hiking and mountain biking.

Carl Turner, DO

Carl Turner, DO

Board Certified in Family Medicine

Bringing smiles, stethoscopes, and sometimes balloon animals to the exam room, Dr. Turner is passionate about providing care in a welcoming, family-focused environment. From newborn checkups to well-woman exams to grandpa’s blood pressure, Dr. Turner enjoys caring for every member of the family. Originally from Bountiful, Utah, Dr. Turner completed undergraduate studies at Weber State University before heading to Des Moines University for medical school. He then trained at Wesley Medical Center in Wichita, Kansas. He has over a decade of experience in primary care and has enjoyed serving families in the rural community of McPherson, Kansas, for the past eight years where he focused on pediatric care and women’s health. He and his wife enjoy the noise of their five wonderful children, three girls and two boys. Outside of the clinic, Dr. Turner loves watching and playing sports, spending time withhis family, and being a part of the community.

Stacy Sumpter, DNP-C

Stacy Sumpter, DNP-C

Doctor of Nursing Practice, Board Certified Nurse Practitioner

Stacy Sumpter, DNP-C began her nursing career in 1994. Living in a rural community, she had the opportunity to work in multiple areas including obstetrics/gynecology, labor and delivery, intensive care unit, medical surgical in addition to home care and hospice. Stacy expanded her education earning a Master’s Degree in Nursing from Graceland University. She began working as a board-certified nurse practitioner in Southern Utah in 2005. Stacy earned her Doctor of Nursing Practice degree from the University of Nevada-Las Vegas in 2022. She took her first course in Bio-identical hormone replacement in 2009 which sparked a passion to learn more and she became certified in Advanced Bio-identical Hormone Replacement Therapy. Stacy believes that education, understanding and partnership are the key components to a successful patient relationship. Stacy enjoys working together to develop a personalized plan of care for health, prevention and disease management. She is a member of the American Academy of Nurse Practitioners, Sigma Theta Tau Zeta Kappa (changed) chapter, and Golden Key International Honor Society and Certified in Advanced Bio-identical hormone replacement therapy. Stacy is married and has three grown children. Together they enjoy boating, trail riding, hiking, and any outdoor activity that Southern Utah has to offer.

Adam Dye, FNP-C

Adam Dye, FNP-C

Master’s Degree in Nursing, Board Certified Family Nurse Practitioner

Adam Dye, FNP-C began his career as a nurse in 2006. He graduated with a BSN from Idaho State University, in Pocatello. Right out of school he started working as an ICU nurse in Idaho Falls. This provided a great opportunity to learn to manage critically ill patients. In 2010, he decided to pursue a master’s degree in Nursing. During that same year, he continued doing school full-time and continued working full-time. Adam graduated in 2012 with a Master’s of Science Degree as a Family Nurse Practitioner, from Idaho State University. Adam was board certified shortly thereafter. Adam enjoys serving his community and getting to know people individually. Adam takes a personal interest in each of his patients’ care and well-being. Adam loves spending time with his wife and six kids. With 4 kids and a set of twin boys, they are sure to keep him on his toes. He enjoys mountain bike riding, running, and staying active and is always up for a fun adventure with family and friends.

Jacob Ewell, DNP-FNP

Jacob Ewell, DNP-FNP

Doctor of Nursing Practice, Certified Family Nurse Practitioner

Jacob Ewell grew up in Southern California. He received his bachelor’s in nursing at Brigham Young University-Idaho and worked as a registered nurse in the Neurological Critical Care Unit at the University of Utah hospital for five years. While working as an ICU nurse, he attended Weber State University where he graduated with his doctorate of nursing practice with an emphasis in family practice. Jacob is passionate about providing preventive and holistic care to patients and their families. He and his wife enjoy traveling and spending time outdoors with their son. Other hobbies of Jacob’s include running, hiking, mountain biking, and dirt biking.

Noah Yoshida, PA-C

Noah Yoshida, PA-C

MPAS, PA-C (Master of Physician Assistant Studies, Board Certified Physician Assistant)

Noah Yoshida was born and raised in Omaha, Nebraska. He received a Bachelor of Science degree in biophysics from Creighton University. After graduating, he was a member of the clinical research team at the University of Nebraska Medical Center and worked in the oncology, hematology, and infectious disease departments. It was at this job that he discovered his passion for patient care and pursued a career as a physician assistant. He received his Master of Physician Assistant Studies degree in 2024 from the University of Nebraska Medical Center. His favorite clinical rotations were dermatology, plastic surgery, oncology, and cardiology. Noah moved with his wife to Saint George in 2023 to support her medical school journey at Rocky Vista University College of Osteopathic Medicine. Aside from helping his wife study for exams, Noah enjoys spending too much time at the gym, exploring the outdoors, discovering new recipes to cook, and salsa dancing. Noah, his wife and their son love hiking around Saint George.

My provider is Dr. Jonathon Baza at one Health located on 292 S 1470 E. St George. Suite 200 St. George, UTAH Dr Baza is an amazing doctor, he’s always making sure that for every visit he will make sure to help his patients to feel better after a visit. He takes his time to chat with the patients regarding our health per each visit even if he just had seen you last just a week or two ago. His professionalism and care towards his patients is unmeasurable. Dr Baza has been my blessings for the past 5 years. He has an amazing staff the MA’s and PA’s are caring and very knowledgeable. The front desk takes care of your appts , calls and check-in appts. I’ve had the pleasure to have been helped by these wonderful ladies at the front desk: Elizabeth , Alivia, Tori and Alicia are amazing when you come for your appointment. Let’s not forget the Lab… painless shots and blood test drawn swiftly and painlessly as well. I also notice that the office is always clean and the exam rooms are impeccably clean and so are the Restrooms. I can go on and on and on about One Health’s amazing patient health care needs. One Health is by far one of the best medical facilities out there. The doctors are very compassionate and caring. I never feel rushed and the care is thorough and comprehensive. The entire staff makes you feel welcomed. I feel fortunate to be a patient of Dr. Jonathan Baza of One Health but all other providers I have seen are professional and dedicated to providing the best care possible.

Dr. Jonathan Baza is difficult to work with. He removed my online account and banned me from the clinic because I expressed displeasure to his M.A. with his repeatedly mishandling of my prescription refills. I was very happy for 5 years with Dr. Tyler Rencher who ended up transferring somewhere else. So I was put with Dr. Jonathan Baza about 2 years ago. He was OK at first but when giving physicals very brief, not thorough, hardly ever looks in my ears nose, throat, never checks reflexes on knees, etc. Just does the bare minimum of stethoscope to heart and lungs. Biggest problem developed getting refills for my 3 main prescriptions, 1 for gastric reflux issues and 2 for BP meds. He would give me a prescription but no refills. And if you haven't been in within 6 months he requires a visit. I have never had any doctor hold essential meds hostage like this. I was due for 90 day refills from Express Scripts for the 3 main daily pills and since Baza gave me none on file, I asked Express Scripts to request him for 3 new 90 day prescriptions for the 3 meds plus 1 renewal for each. It was just barely 6 months since my physical. Regardless he wrote them each for 30 day supply of 30 pills with NO refills. Express scripts proceeded against my knowledge and desires, claiming the drugs were already shipped and billed to my credit card. This cost me $14.00 each when Express Scripts could have given me 90 instead of the 30 he wrote, for the same price. I was not happy. And because he wrote them for 30 and NO REFILLS, I had to go back to him again. He is high maintenance. It is like he's trying to get an extra unnecessary office visit and punishing me if I don't. Because I complained he banned me from the clinic and deleted my online access so I couldn't even write his boss Dr. Paradella to complain. I've never had a doctor so unwilling to provide me at least one refill to make things easier. Generally I could go months not needing to see a doctor, I told them I'm willing to come in every six months yet I only got 30 days of pills and NO REFILLS. Ridiculous. Update. Dr. Baza nor anybody else ever called me to explain why it was necessary to drop me as a patient. I was willing to see other providers there. He did not explain what I did that deserved this. There was no confrontation. One special note: he is not an MD, he is a DO. Those tend to be those who can't get into medical school so they take the osteopathy route instead. I'd prefer an NP or PA over a DO and even expressed desire to switch to a highly respected NP in his clinic, glad to drop him but he is so vindictive he banned me from the clinic. I only relayed messages to him via his M.A. and he is such a coward to not call me. It is like he's afraid to talk man to man. I did not seek any confrontation, I just wanted to understand why he was so stingy with refills which would have helped me without so much unnecessary requests for him.

I highly recommend One Health formerly Dixie Primary Care. From the receptionists and all other staffs are very nice and well mannered. Dr Grace Paradela is my primary Physician. I love her because she always listen to my concern and give a good advise. Thank you so much Dr. Paradela.

I recommend One Health in St George, Utah for a variety of reasons. Docs take their time with patients and actually listen to concerns. And, they offer a variety of wellness treatments to be proactive in preserving great health, especially as we enter our later years. Having just moved here as COVID hit (and I have asthma so I needed a doc right away), I was lucky to find great healthcare providers at One Health. Looking forward to their new building (under construction) and staying with them for the rest of my years.
